By submitting this form, I, the parent/guardian of the players named hereafter, agree that the player’s and I will abide by all rules and regulations of Arizona Sports Coalition. In consideration of the player's participation in the sports programs and activities of the Arizona Sports Coalition, I, for myself and the player’s, hereby release and indemnify Arizona Sports Coalition and affiliates, the owners and operators of the facilities used for the program from and against all claims, liabilities, damages or causes of action arising with the players participation in Arizona Sports Coalition. I also unerstand that the registraiton is not accepted until full payment has been made and that there are no refunds under any circumstances.
